Health & Major Medical Insurance
Employee premium is employer paid. Dependents may be added at employee's expense.

Dental Insurance
Employee premium is employer paid. Dependents may be added at employee's expense.

Term Life Insurance
Employee premium is employer paid. Employees are provided with a $50,000 term life insurance policy. Additional amounts of term life insurance are available to eligible employees, spouses and dependents at employee's expense.

Short Term Disability Insurance
Employee premium is employer paid. Employees are provided with short term disability insurance covering 60% of their salary up to $1,000 per week for a 13 week period.

Long Term Disability Insurance
Employee premium is employer paid. Employees are provided with long term disability insurance covering 60% of their salary up to $6,000 per month.

Supplemental Benefits
An extensive option to purchase additional coverage through independent providers for employees and their dependents is available at employee's expense.

Retirement 401(k) Plan
